How to Change Your Health Insurance Provider in Germany: A Practical Guide for Freelancers and Directors
A stethoscope coiled on a dark green surface with dramatic spotlight lighting, representing the process of switching health insurance providers in Germany for freelancers and directors
Last updated on the 14/09/2026
Switching health insurance in Germany is more straightforward than most people expect, but the timing and the process differ depending on whether you are an employee, a self-employed Freiberufler, or a GmbH director. Getting either wrong can delay your switch by months or leave you temporarily in the wrong plan.
This guide covers how the switch works, what the deadlines are, and what freelancers and directors specifically need to know.
The standard process: what applies to most people
The standard binding period in Germany is twelve months. Once you have been with your current Krankenkasse for at least twelve months, you can switch with two months notice to the end of the month.
The practical timeline works as follows. If you submit your application to your new Krankenkasse in March 2026, the switch takes effect on 1 June 2026. The two-month notice period begins from the first of the following month.
You do not need to cancel your old Krankenkasse yourself. The new Krankenkasse handles the cancellation with your old one automatically. You apply to join the new provider, they notify the old one, and you receive two confirmations: a cancellation notice from the old Krankenkasse and a membership certificate from the new one.
The Sonderkündigungsrecht: switch early if your contributions increase
If your Krankenkasse raises its Zusatzbeitrag, you have a Sonderkündigungsrecht. This special right allows you to switch even if you have not yet completed twelve months of membership.
Your Krankenkasse must notify you in writing at least one month before the increase takes effect, and must explicitly point out your right to switch. From that notification, you have two months to exercise the right. The two-month notice period still applies, so the switch is not immediate, but the twelve-month binding period falls away.
The average Zusatzbeitrag in 2026 is 2.90 percent. If your Krankenkasse’s rate exceeds this, they must also inform you that a cheaper option may be available.
The Sonderkündigungsrecht is worth monitoring closely each January, when most contribution rate changes take effect.

How long should I wait for a response?
Once you submit your application to the new Krankenkasse, acknowledgement should arrive within a few days. The formal membership certificate and cancellation confirmation typically arrive within two to four weeks.
If you have not received confirmation from the new Krankenkasse within three weeks of submitting your application, follow up in writing. Keep a record of the date you submitted. The switch does not take effect automatically if the paperwork has been lost or delayed, so it is worth confirming before your intended start date.
What freelancers and directors need to know
The standard process above applies to employees. For self-employed Freiberufler and GmbH directors, the situation is different in several ways.
You are a voluntary member, not a compulsory member. Freelancers and self-employed individuals who choose to remain in the GKV are voluntarily insured. The same notice periods and binding rules apply as for compulsory members.
Your contributions are calculated differently. For freelancers, the Krankenkasse calculates contributions provisionally based on the most recent income tax assessment. Once the final assessment for the relevant year is available, the contribution is corrected retrospectively. This means your monthly contribution may change retroactively once your Steuerbescheid arrives.
The Mindestbemessungsgrundlage applies. In 2026, the minimum assessment base is €1,318.33 per month, resulting in minimum monthly contributions of approximately €260 to €278 for voluntarily insured individuals, regardless of actual income. If your income is below this threshold, you still pay based on the minimum.
The three-month deadline for new freelancers. Freelancers who were previously compulsorily insured and wish to continue in the GKV voluntarily must apply within three months of the end of their compulsory membership under § 9 Abs. 2 SGB V. Missing this window can force a switch to private insurance.
The PKV threshold. The Jahresarbeitsentgeltgrenze in 2026 is €77,400 gross annually. Above this level, employees are free to switch to private health insurance. For self-employed individuals, the option to switch to private insurance exists regardless of income level.
Switching from PKV back to GKV is difficult. If you have already chosen private health insurance, returning to the GKV is only possible under specific conditions. For most self-employed people, once you have moved to PKV, the route back requires a change in employment status (for example, taking an employed role below the income threshold) or a significant change in circumstances. This is one of the most consequential decisions a freelancer or director makes, and it should not be made based on short-term cost comparisons alone.
What to compare before switching
Not all Krankenkassen charge the same. The base contribution rate is set centrally, but each Krankenkasse sets its own Zusatzbeitrag on top. In 2026, this ranges from below 1.5 percent to over 3 percent depending on the provider.
Beyond the contribution rate, the differences between providers include supplementary benefits (Zusatzleistungen) such as coverage for alternative treatments, dental provision, vision care, and health bonuses. Service quality and ease of contact also vary significantly, which matters when you need to submit documents or follow up on claims.
For freelancers, the administrative experience of the Krankenkasse with self-employed members is worth considering separately. Not all providers handle the voluntary contribution calculation process or the income change notifications equally well.

Step by step: how to make the switch
Step 1: Check when you joined your current Krankenkasse. If it has been at least twelve months, you are eligible to switch. If not, check whether a Sonderkündigungsrecht applies due to a recent contribution increase.
Step 2: Compare providers using a comparison platform such as krankenkassen.de or the GKV Spitzenverband’s list. Focus on Zusatzbeitrag, Zusatzleistungen, and service reputation.
Step 3: Submit your application to the new Krankenkasse. Do this by the end of the month you want the notice period to start from.
Step 4: The new Krankenkasse notifies your old one. You do not need to write a separate cancellation letter.
Step 5: Receive your two confirmation documents: cancellation notice from the old provider and membership certificate from the new one.
Step 6: If you are an employee, submit the new membership certificate to your employer so contributions are directed to the correct provider. If you are self-employed, update your records and any standing payment arrangements.
Common mistakes
Waiting too long after a Sonderkündigungsrecht notification. The right to switch expires after two months from the date of notification. If you miss it, you return to the standard twelve-month binding period for that cycle.
Applying too late in the month. If you submit your application on the last day of the month but it does not reach the new Krankenkasse until the following day, the notice period shifts by one month. Allow a few days’ margin.
Assuming the switch is confirmed without checking. The process is largely automatic, but documentation can go missing. Always confirm receipt of both the cancellation notice and the new membership certificate before your intended start date.
For freelancers: switching during a year when income is significantly different from the previous tax year. If your income changes substantially, your provisional contribution will not reflect your actual earnings until the retrospective adjustment. This can result in a significant back-payment or refund, depending on whether income went up or down.
Frequently asked questions
How long must I wait before I can switch Krankenkasse? After twelve months of membership with your current provider, you can switch with two months notice to the end of the month.
Do I need to cancel my old Krankenkasse myself? No. The new Krankenkasse handles the cancellation on your behalf once you submit your application to them.
Can I switch Krankenkasse as a self-employed Freiberufler? Yes. The same twelve-month binding period and two-month notice rules apply to voluntarily insured freelancers as to employed members.
What is the Sonderkündigungsrecht? A special right to switch outside the normal twelve-month binding period, triggered when your Krankenkasse raises its Zusatzbeitrag. The two-month notice period still applies.
Can I switch to private health insurance at any time? As a self-employed individual or Freiberufler, you can switch to private health insurance regardless of income level. The decision should be made carefully, as returning to the GKV afterwards is restricted.
What is the minimum monthly contribution in the GKV for freelancers in 2026? The minimum assessment base in 2026 is €1,318.33 per month, resulting in minimum monthly contributions of approximately €260 to €278 for voluntarily insured individuals, depending on the specific Krankenkasse and its Zusatzbeitrag rate.
The bottom line
Switching Krankenkasse in Germany is a well-defined process with clear timelines. The standard route requires twelve months membership and two months notice. The Sonderkündigungsrecht removes the twelve-month requirement if your provider raises its rates. The new provider handles the cancellation.
For freelancers and directors, the additional considerations are the voluntary membership calculation, the three-month window when starting self-employment, and the long-term consequences of any move toward private insurance.
